Rapid Hospital Discharge Program
Recruit, Connect & Care accept NDIS participants who are in need of urgent hospital discharges. We have assisted many participants with a transition from hospital to Short Term Respite (STR), Medium Term Accommodation (MTA) or into a Supported Independent Living (SIL) property such as a shared residential house, Specialty Disability Accommodation (SDA) or back into their own homes.
We understand that the hospital transition is a challenging and overwhelming time, so we have fine-tuned our processes to ensure that your discharge is as smooth as possible.
Here are some things that we do when a participant is being discharged from the hospital:
- We work with the teams at the hospital to ensure that we have a thorough understanding of your health and ways to prevent re-admission.
- We work with your Support Coordinator to make sure we are connected to all of the relevant people in your life such as your personal support system and your allied health team.
- We learn about who you are and how you like to go about your days.
- We support you with a team of support workers who are experienced with hospital discharges and are familiar with the challenges, anxieties and uncertainty that can come with a hospital discharge.
- We let our support workers know who you are, how you like to be supported, and how best to support you.
- We source accommodation for you and find somewhere for you that you will be able to stay for a while.
- We book a post-discharge appointment with your doctor and support you in attending this appointment.
- We work with you, your allied health and your loved ones to ensure that you are being supported to live your life in a way that is healthy, safe, and true to both you and your goals.
Hospital discharges can be tough. When we are contacted, we understand that sometimes the pace of the transition has to be very fast. We know that people are often coming into a new phase of their life where they require more support and that this can be confronting.
